Consent to engage in AsproOffice services *

I consent for Asprodental to send insurance claims and enter insurance payments on behalf of my dental practice. To ensure best results, I agree to maintain accurate insurance coverage information on my patients and to upload paper EOBs into the shared folder on Google Drive. I acknowledge that AsproOffice does not check for insurance eligibility, breakdowns, fee schedules, or follow up on statements to my patients.

Consent to payment for services *

I consent for Asprodental to process my card on file for 3% of insurance collections. Payment will be processed on the first of each month for the prior month’s services.

Cancellation policy *

I may discontinue AsproOffice services at any time with a written cancellation request. Asprodental will collect payment on the day of termination for any unpaid services rendered up to the date of cancellation. If payment is not processed within 3 business days from the first of the month, Asprodental reserves the right to discontinue the AsproOffice insurance billing and posting services and will require immediate payment for services rendered.
